What is melatonin?

Melatonin is classified as a medicine
In the UK, however, we have been thinking very intensively about melatonin since the 1990s. In the USA, melatonin is available as food in all supermarkets, drugstores or even at gas stations, without somehow requiring pharmaceutical advice or even requiring a doctor’s prescription. You just take it off the shelf there and you can buy it uncontrollably.
In the UK, in 1996, the relevant ministry decided that melatonin is a substance with pharmacological effects and that it must therefore be classified as a medicinal product. As a result, from that point on, all products had to be approved by the Ministry of Health in the UK via a drug approval process, which means that appropriate studies on efficacy, safety and quality must also be submitted. Thus, preparations containing melatonin as a dietary supplement were no longer marketable because they do not have this approval and carrying out the necessary studies involves significant costs.
Melatonin in the USA
In the USA, a decision on melatonin was made in the 1990s, but it was much different. Melatonin has been classified as food. The US seemingly interpreted the study situation differently at the time and saw classification as a medicine as not given. The legal situation in the US stipulates that the competent authority would later have to prove that products are harmful to the body after all, as unexpected side effects may occur above or so. This is the only way to make a different classification. As a result, melatonin has been buying in the US for decades without a prescription. Governments have also come up with different classifications in some European countries than the UK, the Netherlands and Spain you can buy melatonin over the counter. There is no single European classification for such substances, but it is up to the countries to decide.
Melatonin on doctor’s prescription
It took until 2008 in the UK for the first medicine with the active ingredient melatonin to actually be approved. This is the ready-to-use medicine circadin, which contains two milligrams of melatonin. The approval has also been pronounced only on one indication, the short-term treatment of severe sleep disorders in people over 55 years of age. In addition, treatment has also been limited to 13 weeks.
In 2012, the European Food Authority (EFSA) looked at the so-called health claims, the health claims that manufacturers are allowed to make with their food products. This includes dietary supplements. The Health Claim clearly dictates which advertising statements are legally permitted and which information may be used to advertise on the Internet, on brochures, in advertising or on the product itself. This EFSA awarded melatonin two such health claims to melatonin in 2012. On the one hand, the short-term treatment of jet lag and on the other hand the shortening of the sleep phase for one and during sleep disorders.
Approved health claims for melatonin
This has led to the assumption on the manufacturer side that the classification as a medicinal product is invalid if the health claims are adhered to and used in conjunction with your products. Manufacturers claim that we can intake the same amount of melatonin through food in a very natural way as is contained in these products, and that they must therefore not be classified as medicines. Legal proceedings are currently under way here and I am curious to see what verdict the judges will reach, because this could greatly liberalise the trade in melatonin in the UK and render drug tourism from abroad obsolete.
In the UK guidelines on the treatment of one and sleep disorders, which were recently newly adopted, melatonin plays only a very minor role. Luckily, however, one finally distances oneself from the classic sleeping pills and only gives them a benefit for short-term therapy. The first and most important measure is therefore behavioural therapy with appropriate relaxation measures and good sleep hygiene. Melatonin is mentioned only very briefly on the sidelines, because here the responsible scientists do not see a meaningful study situation.
In my opinion, this weak significance of the study situation after so many years of application in other countries is actually very incomprehensible. Apparently, you still don’t know enough about normal blood levels or reference values. The dosage of melatonin supplements is also still being discussed. There are very different dosed products from 0.5 mg to 5 mg.
High dosage of melatonin is more likely to be unsuitable
If one reads on the part of US doctors on this topic, the high dosage is sometimes viewed quite critically. Here, a dosage of one to two milligrams crystallizes.
One trend that can be observed in the US is melatonin administration in children. This is viewed very critically by experts. Parents believe that through melatonin they do something good to their children to send them fitter to school in the morning or to increase and improve learning processes.
This development is, of course, also reflected in marketing, which is much more unrestricted in the United States and is certainly not permissible in Europe in this form. I take a critical view of this trend, because many manufacturers are exploiting consumer ignorance in order to optimise their sales and health usually plays a very minor role in this.
Since melatonin can be sold in many countries without the approval of medicinal law, the pharmaceutical industry probably has only a small interest in financing studies and sending products through these procedures, so that in the UK also in this regard little will be done in the coming years if there will be no changes in the jurisprudence. Drug tourism will therefore continue to be high for such substances, as many people organise the desired products from abroad.
